# What documents are accepted for bank account or debit card redemptions?

Redemptions to a bank account or debit card sometimes need extra documentation. If yours does, upload it right to your [Verifications](https://modo.us/my-profile?section=verifications) page.

All documents must:

*   Be dated within the last 12 months
*   Match the full name on your Modo account
*   Meet the requirements below for your document type

*   Photo of your physical card 

Must show your name, expiration date & the last 4 digits of your card number. _Cover the rest of your card number with a physical object_

****Proof of Bank Account****

*   Bank statement 
*   Voided check
*   Official document from your bank

Must show your name and the last 4 digits of your account number. Screenshots not accepted, must be a PDF file

****Cash App Proof****

*   Full account statement (downloaded PDF)

We need the entire statement document in order to show your name (top of page 1) and last 4 digits of your account number (bottom of final page)

****PayPal Direct Deposit Form****

*   Direct deposit form

Modo as the company, and e-signed by you
